At your earliest convenience, an Agent will visit your home to begin the qualification process.

The Agent will fill out a Health Risk Assessment (HRA) form and discuss personal and family health history, your current health situation and any medications you may be on. They will then send all documents to your primary care physician (PCP) for review and approval.

Using a mouth swab, a sample of your saliva will be taken and sent to one of our laboratories for testing.

1. Meet with one of our Agents.
At your earliest convenience, an Agent will visit your home to begin the qualification process.
2. Answer simple questions. Documentation will be sent to your PCP.
The Agent will fill out a Health Risk Assessment (HRA) form and discuss personal and family health history, your current health situation and any medications you may be on. They will then send all documents to your primary care physician (PCP) for review and approval.
3. A saliva sample is collected using a mouth swab.

Using a mouth swab, a sample of your saliva will be taken and sent to one of our laboratories for testing.

4. Your and your PCP are sent the results.
10 to 12 weeks later you and your PCP will receive your results in the mail.
